Open reduction and internal fixation versus resection for radial head fractures: a meta-analysis
نویسندگان
چکیده
Purpose: Radial head resection and open reduction and internal fixation (ORIF) are the traditional techniques for the treatment of radial head fractures (RHFs). The existing literature has not identified which is superior. We performed a meta-analysis of the current literature on excision and ORIF for RHFs to reach a relatively conclusive answer. Methods: A search was performed in PubMed, OVID, EMBASE, and Cochrane Library for articles published on RHFs. We extracted and pooled patient data using standard meta-analytic approaches. The outcomes were compared using measures of functional rating score, grip strength, range of motion, pain score, physicianrated questionnaire, and complication rates. Outcomes were pooled by random-effects if high heterogeneity was detected. Sensitivity analyses were used to test the stability of the meta-analysis results under different assumptions. Results: Four studies involving 162 patients were considered eligible for analysis. Results showed ORIF was more effective for avoiding flexion contracture and degenerative change complications. For functional outcome (The American Shoulder and Elbow Surgeons Elbow Evaluation Instrument, The Broberg and Morrey functional rating score, The classification rating, The patient-based Disabilities of the Arm, Shoulder, and Hand questionnaire), ORIF had a superior capability to resection. As for the loss of grip strength, the arc of flexion/extension of the elbow and pronation/supination of the forearm, pain relief, and radiographic proximal migration of the radius, there were no significant differences between two interventions. Conclusions: To sum up, considering all these pooled outcomes, we recommend ORIF over Resection for the treatment of RHFs.

The Best Option in Treatment of Modified Mason Type III Radial Head Fractures: Open Reduction and Internal Fixation Versus Radial Head Excision
Background: Radial head fractures commonly occur during elbow traumas. Among those, treatment of Mason type IIIfractures is still under controversy. Common treatment methods for these fractures include open reduction and internalfixation (ORIF) as well as radial head excision. In this study, we compared long-term outcomes of both methods intreatment of patients with Mason type III fractures of ...
متن کاملRadial head fractures.
Radial head fractures are the most common adult elbow fracture. Our ability to treat these fractures has improved with increased understanding of elbow biomechanics. Based on fracture type, possible treatment includes nonoperative management, open reduction and internal fixation, radial head resection, and replacement arthroplasty. Management and operative technique are discussed.
متن کاملOpen Reduction and Internal Fixation of Complex Radial Head Fractures
Fractures of the radial head have an incidence of about 1.7% to 5.4% of all adult fractures. These fractures are classified as simple or complex, depending upon whether there is an associated fracture or a ligamentous injury. Thorough clinical & radiological assessment should be done to differentiate simple fractures from complex fractures with concomitant elbow injury. The management of comple...
متن کاملFunctional Outcome of Internal Fixation of Radial and Ulna Fracture
Background: Displaced fractures of radius and ulna bones in adults are usually treated with open reduction and internal fixation. The purpose of this research was to access functional outcome of these fractures. Materials and Methods: In this research 52 Patients who underwent ORIF with DCP 3.5 in past 5 years were investigated. Post-operation protocol was one week immobilization and then acti...
متن کاملHardware Removal Due to Infection after Open Reduction and Internal Fixation: Trends and Predictors
Background: Little is known about trends and predictors of hardware related infection following open reduction and internal fixation (ORIF) of extremity fractures, one of the major causes of failure following ORIF. The present study was designed and conducted to determine trends and predictors of infection-related hardware removal following ORIF of extremities using a nationally representative...
